Detection tuning fails when it is treated as a one-time cleanup rather than a standing loop. The version that works looks like this: measure the alert's precision over a fixed window, find the single field that separates signal from noise, narrow on that field only, and re-measure.
What matters is that every tuning change is reversible and recorded. A rule that was narrowed six months ago for reasons nobody wrote down is a rule nobody will ever widen again.
